We are seeking a dedicated Home-Based Clinical Therapist to join our team. As a Home-Based Therapist, you will provide essential support and therapeutic services to families in their homes, empowering them to achieve positive change.

What You'll Do:

Assess family needs through interviews and approved assessment tools, considering social history and collaborating with community resources.
Develop and implement individualized treatment plans with specific, measurable goals and objectives.
Provide a range of therapeutic interventions, including individual, family, and group counseling sessions, all within the comfort of the family's home.
Offer case management services as needed, connecting families with vital resources within the community and the county's support system.
Collaborate with other agencies and parties involved in the family's care, ensuring a coordinated and comprehensive approach.
Participate in multidisciplinary case staffing to provide integrated and holistic care.
Maintain accurate and timely records and documentation of all services provided.
Prepare reports as requested, contributing to effective communication and collaboration.
Engage in crisis intervention when necessary, providing support and guidance during challenging situations.
Adhere to all safety protocols and procedures, ensuring a safe and supportive environment for families.
Perform other related duties as assigned.
What You'll Bring:

A Master's degree in counseling, social work, or psychology.
Experience providing individual, family, and group therapy to diverse populations.
Experience working with youth and families, particularly those involved in the juvenile justice system, is a plus.
Reliable transportation, a valid driver's license, and auto insurance are essential for conducting home visits.
Licensure/registration within your defined area.
Strong computer skills and the ability to use electronic health records.
Excellent written and verbal communication skills.
