Schedule : 40 hours per week | Tuesday, Wednesday, Thursday | 11 am – 7:30 pm | Alternating Weekend & Holiday Rotation- Required

LifeCircles PACE is seeking a compassionate and dedicated Home Health Aide (HHA) to support older adults in living safely, comfortably, and independently in their own homes. In this rewarding role, you’ll provide essential hands-on care and meaningful support that helps preserve dignity, enhance quality of life, and bring peace of mind to participants and their families.
Key Responsibilities
- Provide Hands-On Care – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
- Support Mealtime– Help (residents or participants) enjoy their meals and stay hydrated for good health.
- Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
- Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
- Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
- Provide Responsive Support – Answer call lights quickly and respond with kindness and compassion.
- Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
- Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

- Certified Home Health Aide (HHA) or Certified Nurse Aide (CNA) per state requirements, required.
- Previous experience providing care to older adults, preferred.
- Reliable transportation and valid driver’s license, required.
- High school diploma or GED, required.
- Excellent communication and observation skills.
