Position Overview
We are seeking experienced, highly reliable direct care professionals to support adults with severe Traumatic Brain Injury (TBI) in a structured, home-based setting. This role goes beyond traditional caregiving and requires strong skills in behavioral support, cognitive cueing, and hands-on physical care.
This is an ideal role for candidates with backgrounds in neuro rehab, skilled nursing, behavioral health, or high-acuity home care.
Key Responsibilities
- Provide direct care for individuals with significant cognitive and physical impairments
- Support patients with all activities of daily living (ADLs), including total assist and incontinence care
- Utilize behavioral management techniques, redirection, and de-escalation strategies
- Assist with safe transfers (gait belt, Hoyer lift, etc.)
- Monitor for changes in condition and communicate effectively with the care team
- Administer and document medications and treatments as prescribed by physicians and/or health care professionals. Chart each client’s daily documentation log including goals, completed tasks, and behavior before the end of your shift.
- Maintain a structured, safe, and supportive environment
- Cook/prepare and serve meals according to a set menu and adherence to prescribed diets
- Moderate housekeeping/cleaning duties are assigned daily. Must follow Healthcare Facility guidelines regarding cleaning, disinfecting, and infection control protocols to ensure facility, client, and visitor areas remain in clean order at all times.
Required Qualifications
- Minimum 1–2 years of experience in one or more of the following:
- Neuro rehabilitation
- Skilled nursing facility (SNF)
- Behavioral health / psychiatric care
- High-acuity home care
- Comfort with high physical care needs, including total assist and incontinence
- Ability to manage challenging behaviors such as impulsivity, agitation, or verbal outbursts
- Dependable, professional, and able to follow structured care plan
- Valid Driver's License with less than 3 points
- CPR and First Aid certification
- Able to pass extensive background check including fingerprinting
Preferred Qualifications
- CNA (Certified Nursing Assistant) or Home Health Aide (HHA)
- Experience working with brain injury, stroke, or neurological conditions
- CPI (Crisis Prevention Intervention) or similar training
Important Considerations
This role requires patience, consistency, and a strong understanding of cognitive and behavioral challenges associated with brain injury. Candidates must be comfortable working in environments where patients may demonstrate impaired judgment, resistance to care, or fluctuating behaviors.
