We are seeking a Home Care Supervisor to provide frontline leadership, ensure high-quality care, and support our dedicated staff in delivering compassionate care to participants in both home and community care settings. In this role, you'll play a crucial part in shaping the care we provide, fostering a collaborative and positive team environment, and ensuring the highest standards of participant well-being.
What You'll Do:
- Lead and mentor a team of Certified Nurse Aides and Personal Care Workers in delivering quality care to participants in various settings, including homes and care facilities.
- Supervise scheduling to ensure great care coverage.
- Complete supervisor home visits in our 7 county service area.
- Provide hands-on leadership for staff training, performance management, and team building.
- Assess and respond to participant condition changes, ensuring timely and accurate documentation in the medical record.
- Collaborate with interdisciplinary teams to provide care that enhances the well-being of our frail or elderly population.
- Foster positive relationships between staff and departments, implementing changesto improve care goals and operations.
- Ensure ongoing staff education, skill development, and compliance with regulations.
- Conduct audits and ensure key performance indicators and quality measures are met.
What We’re Looking For:
- Education: High School Diploma or equivalent. Preferably, an Associate or Bachelor’s Degree in Nursing (ADN or BSN).
- Licensure/Certification: Valid Michigan RN license, CPR certification, and a valid driver’s license with auto insurance.
- Experience: 3-5 years of experience in home or community health care, including at least 1 year of working with a frail or elderly population.
- Leadership Experience: Previous successful leadership experience in a healthcare setting is preferred.
- Skills: Ability to lead with compassion and professionalism, excellent communication, problem-solving, and organizational skills, and a strong understanding of clinical competencies for geriatric care.
