Home Health Rehabilitation Manager - Physical Therapist (PT), Full Time,

As a Rehab Manager/Physical Therapist with AdvisaCare, you will have the opportunity to work with diverse patients, build/lead multidisciplinary teams, and develop tailored treatment plans that cater to unique patient needs.

In this role, you will conduct comprehensive assessments, implement effective therapy techniques, and monitor progress to ensure the best possible outcomes for your clients.

Key Responsibilities:
- Perform in-home assessments and develop individualized treatment plans based on patient needs.
- Provide one-on-one therapy sessions and implement treatment protocols to enhance physical function.
- Regularly evaluate patient progress and adjust treatment plans as necessary to ensure effective outcomes.
- Educate patients and their families about treatment options, home exercises, and wellness strategies.
- Document patient assessments, treatment plans, and progress in accordance with company policies and regulatory requirements.
- Collaborate with healthcare teams to ensure comprehensive patient care and communication.
- Review/support team members in his/her rehabilitation work, across multiple disciplines.
Requirements
Qualifications:
- Must hold a valid and active Physical Therapist license in the state of Michigan.
- Experience in a home health or rehabilitation setting is highly preferred.
- Strong clinical skills and a deep understanding of physical therapy practices.
- Excellent communication abilities and a patient-centered approach.
- Ability to work independently and manage time efficiently in a home setting.
- Valid driver’s license and reliable transportation for traveling to patient locations.
