Job Description

Proactive Hospice Care is seeking a compassionate and experienced Chaplain to join our interdisciplinary hospice team. The Chaplain will provide spiritual and emotional support to patients, their families, and caregivers, offering comfort and guidance aligned with their unique beliefs and preferences. This role is essential in ensuring that patients and their loved ones feel supported and at peace during their end-of-life journey.

Key Responsibilities

• Provide spiritual care and counseling to patients and families, respecting their cultural and religious preferences.
• Conduct spiritual assessments and develop individualized care plans.
• Facilitate prayer, meditation, rituals, or other practices as requested by patients or families.
• Collaborate with the interdisciplinary team (IDG) to address spiritual and emotional needs.
• Assist families with bereavement support, including memorial services and follow-up care.
• Document all care provided in compliance with hospice regulations and company policies.
• Participate in IDG meetings and contribute to the overall care plan.

Qualifications

• Master’s Degree in Divinity, Theology, Pastoral Counseling, or a related field preferred.
• Ordination or endorsement by a recognized faith tradition.
• Minimum of 2 years of experience providing spiritual care, preferably in hospice or healthcare settings.
• Certification in Clinical Pastoral Education (CPE) is a plus.
• Strong interpersonal and communication skills.
• Ability to work with diverse populations and honor all religious and cultural practices.
