Overview
Registered Nurse – Home Health (Travel) in Battle Creek, Michigan. Start date: March 20, 2026 for a 13‑week assignment with guaranteed 40 hours per week. Weekly pay ranges from $1,750 to $1,757. Responsibilities
- Conduct in-person participant assessments to determine appropriate levels of care per the plan of care, categorizing status as emergent, urgent, or non-urgent for home care.
- Respond promptly to calls from PCPs, triage teams, and SCPP support staff; return calls promptly and document all contacts with follow-up as needed.
- Complete field visits using full-body assessment, critical thinking, and use of EMR; deliver nursing care during regular, routine, acute, and non-acute visits.
- Provide nursing care and treatment as ordered by the PCP, including wound care, IV hydration, lab draws, and comprehensive nurse assessments; educate participants, DPOAs, families, and facility staff.
- Coordinate and collaborate with providers, SCPP staff, DME suppliers, assisted living facilities, SNFs, paramedics, pharmacies, administration, home care coordinators, hospitals, and other contracted partners.
- Initiate emergency treatment when necessary and document new orders as indicated, including updating PUIs.
- Document and report events such as falls, injuries, threats of suicide, medication errors, incident reports, grievances, and other quality measures; maintain participant-centered care throughout all interactions.
- Engage in orientation, ongoing in-service programs, staff meetings, continuous quality improvement, and protocol evaluations; adapt to changing workloads and keep leadership informed; perform other duties as assigned.

Requirements
- Education: RN Nursing Diploma or Degree from an accredited school; Bachelor’s degree preferred; Associate degree required.
- Experience: Minimum of one year working with frail, elderly, or long-term care populations; one year of home care experience preferred; one year of wound care experience preferred.
- Certifications: Possess current CPR certification by the American Heart Association or willingness to obtain as offered.
- Licenses/Registrations: Not specified in the notes; must maintain eligibility to practice as an RN in the applicable location.
- Additional: Must have reliable transportation, a current valid Michigan driver’s license, and car insurance; must be 18 years of age or older; fully vaccinated or willing to be vaccinated against influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
