University Home Care Inc is seeking a compassionate and experienced Physical HomeCare Therapist with Kinnser experience to provide high-quality in-home therapy services in Southgate, MI and surrounding areas. This role involves delivering personalized care plans to help patients improve their physical health and regain independence in the comfort of their own homes. The ideal candidate will be dedicated to enhancing patient outcomes through professional and empathetic care.

• Provide skilled physical therapy services to patients in their homes across Southgate and surrounding areas.
• Develop, implement, and modify individualized treatment plans based on patient needs.
• Accurately document all therapy sessions using Kinnser software.
• Communicate regularly with patients, families, and the healthcare team to ensure continuity of care.
• Monitor patient progress and adjust therapies as needed to achieve optimal outcomes.
• Comply with all clinical and regulatory standards related to home health care.

• Valid Physical Therapist license in the state of Michigan.
• Experience working with Kinnser software is required.
• Proven clinical experience in home health physical therapy.
• Strong communication and interpersonal skills.
• Reliable transportation to travel within Southgate, MI and surrounding areas.
• Ability to work independently and manage caseload effectively.
