Hours: Exempt, Min. 40 hours per week
Schedule: 7 days per pay period with a rotation of 7 days working and then 7 days off
- Monday – Friday: 4:30pm – 8:00 am
- Friday at 4:30pm till Monday Morning at 8:00am.

What You’ll Do in This Role:
The Thome PACE Extended Care Nurse provides after-hours care and weekend coverage for participants, working 7 days per pay period on a rotating schedule (7 days on, 7 days off). Responsibilities include taking nursing calls, assessing needs for emergent visits (4:30 p.m. - 8:00 a.m.), and working scheduled weekend shifts ( 8:00 a.m. - 4:30 p.m. Saturday and Sunday). On-call hours are from 4:30 p.m. Friday to 8:00 a.m. Monday. The nurse collaborates with the Thome PACE Interdisciplinary Team to deliver integrative care to frail elders, focusing on enhancing functional capacity, promoting autonomy, and ensuring coordinated care.
Primary Responsibilities:
- Provide advanced nursing care as per clinical treatment plans and conduct participant visits based on assessments.
- Assess nursing needs through phone or in-person evaluations and address participant concerns, documenting actions taken.
- Perform diagnostic, preventive, and rehabilitative nursing procedures, ensuring safety and proper care, and communicate with the clinical team.
- Monitor participants' conditions and report changes to the clinical team as needed.
- Educate and counsel participants and families on care needs and procedures based on the participant's condition.
- Maintain effective communication and ensure compliance with the participant's Plan of Care.
- Maintain accurate, timely care progress records and submit documentation per organizational standards.
- Follow organizational policies related to service delivery, documentation, and care coordination, ensuring compliance during supervisory visits.
- Actively participate in quality improvement initiatives.
- Effectively, triage nursing calls for participants.
- Collaborate as a key member of the Interdisciplinary Team, attending meetings and contributing to care planning.
- Perform other related duties as required, adapting to organizational needs.
✅ What You’ll Need:
- Must be licensed to practice as a Licensed Nurse Practitioner (LPN) in the state of Michigan.
- Must have a minimum of 1 year of nursing experience.
- Previous Home Health or Hospice experience preferred.
- Must be CPR certified or willing to obtain
- Must have reliable transportation and possess a valid and current driver's license
