Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. We serve all populations regardless of their ability to pay. This position reports to the Director of Care Management and will function as an essential team member for Honor Community Health who advocates for the patient and helps to remove barriers to improved patient health. The Community Health Worker (CHW) engages in collaboration with multiple team members as part of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of social determinants of health needs in the presence of other comorbid physical and behavioral health conditions.
What are we looking for?
- A High school diploma or equivalent is required.
- A High School Diploma or equivalent is required.
- Current MiCHWA Community Health Worker certification or the ability to obtain within 12 months of hire.
- 1+ year of experience working in a clinical environment required.
- Strong computer skills utilizing CHAMPS, Nextgen, and Microsoft Office.
