Supervisor, Integrated Health Services

As an Integrated Health Care Manager, youll play a key role in supporting consumers with co-occurring and chronic conditions. Youll monitor health risks, coordinate care across providers, and ensure that interventions address both physical and mental health needs. This position bridges mental health and primary care, supporting staff in the clinic, engaging consumers in wellness activities, and ensuring individuals receive the education, coordination, and follow-up they need to thrive. What Youll Do Assess & monitor: Conduct screenings, track health status through data platforms, and manage care alerts for moderate- to high-risk consumers. Coordinate care: Collaborate with case managers, treatment teams, and outside providers to ensure whole-health support. Support clinical teams: Provide training to staff and assist in managing appointments at integrated health clinic locations. Promote wellness: Develop and implement wellness programs that encourage healthy habits across the 8 domains of wellness. Facilitate transitions: Support consumers moving from inpatient or acute healthcare settings to the community. Engage consumers: Enroll individuals in physical health programs, assist with labs and assessments, and encourage annual health checks. Drive growth: Help develop and expand integrated health services in both Hillsdale and Jackson. Ensure compliance: Maintain adherence to CARF, MDHHS, and grant requirements. Why Join LifeWays?
