Care Transition RN: Medicaid Care Coordinator
Molina Healthcare Inc. is seeking a Care Review Clinician RN to support Medicaid members recently admitted to a hospital. The role combines phone outreach and in-person meetings during inpatient stays, with travel to local hospitals.
Strong computer skills and meticulous notes are essential to coordinate care from admission through discharge. Preferred candidates have case management or managed care experience; behavioral health experience is a plus.
