Your Core Mission:
As a Home Care CNA, you will be the cornerstone of patient well-being, focusing on comfort, safety, and emotional support.
Key Responsibilities:
- Assisting with bathing, dressing, grooming, toileting, and mobility (walking/transfers)
- Monitoring and recording vital signs and administering medications/treatments as prescribed
- Preparing meals and assisting with feeding, ensuring dietary guidelines are followed
- Performing light housekeeping (laundry, changing linens) to maintain a clean and safe environment
- Maintaining accurate and timely patient records and detailed documentation
- Immediately reporting any changes in the patient's condition to the supervising nurse
- Providing essential companionship and compassionate support to both the patient and their family
Qualifications:
- Must be a Certified Nursing Assistant (CNA) with state-approved training
- Strong verbal and written communication and interpersonal skills
- Proven ability to work independently and manage time effectively
- Proficiency in patient record-keeping and policy adherence
- Physical ability to perform patient transfers and assist with mobility
