LPN (Home Health), Full Time

The Home Health Care LPN will play a critical role in providing quality medical care and support to individuals in their homes. The primary responsibility will be to perform skilled nursing interventions, educate patients on how to manage their health conditions, administer medications, and collaborate with other healthcare professionals to ensure comprehensive care.
Qualifications:
Current and valid LPN license in the state of practice.
Proven experience as an LPN, preferably in a home health care setting.
Excellent clinical skills and a solid understanding of nursing principles and practices.
Strong assessment and critical thinking abilities.
Effective communication and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Compassionate, patient-centered approach to care.
Proficient computer skills for documentation and communication purposes.
Flexibility to adapt to varying patient needs and schedules.
Valid driver's license and reliable transportation.
Pay Range: $18.49 - $27.38/hour
Employment Type: Full Time, 72 hours every 2 weeks

Requirements: Current and valid LPN license in the state of Michigan required, Valid driver's license and reliable transportation required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required
Provide Compassionate Care
- Educate patients on management of acute and chronic health conditions
- Administer medications and treatments as prescribed by physicians. Including but not limited to wound care, ostomy care, tube feeding, peripheral lab draws, and catheter changes.
- Monitor patients' vital signs and assess their overall health condition. Communicating any concerns to the appropriate physician and triage the need for urgent intervention if necessary.
- Help patients manage pain and discomfort through appropriate interventions.
Perform Documentation
- Document accurate and detailed patient information, including symptoms, progress, and any changes in condition.
- Capture communication with the supervising registered nurse or physician and other members of the interdisciplinary team to provide updates on patient conditions and progress.
Educate Patients and Families
- Educate patients and their families about medical conditions, treatment plans, and self-care techniques.
- Provide guidance and support in managing chronic illnesses, medications, and lifestyle modifications.
- Teach patients and their caregivers about wound care, medication administration, and disease prevention strategies.
- Ensure patients and their families understand and adhere to prescribed treatments and therapies.
Collaborate with Healthcare Professionals
- Communicate and collaborate effectively with physicians, registered nurses, therapists, social workers, and other healthcare team members.
- Participate in case conferences and rounds to discuss patient progress, challenges, and adjustments to care plans.
- Seek guidance from the supervising registered nurse or physician when faced with complex patient situations or changes in condition.
- Report any concerns, incidents, or changes in patient status promptly to the appropriate healthcare professionals.
Ensure Compliance and Safety
- Adhere to legal, ethical, and professional standards of practice.
- Comply with all relevant regulations and guidelines, including those related to patient privacy and confidentiality.
- Promote a safe environment by identifying and addressing potential hazards and implementing appropriate preventive measures.
- Maintain accurate and up-to-date patient records and documentation in accordance with organizational policies and procedures.
Perform Wound Care
- Assess and manage wounds, including surgical incisions, pressure ulcers, and other acute or chronic wounds.
- Cleanse and dress wounds using sterile techniques and appropriate wound care products.
- Monitor wound healing progress and identify signs of infection or complications.
- Educate patients and caregivers on proper wound care techniques and prevention of infection.
- Collaborate with the healthcare team to coordinate specialized interventions, such as the use of negative pressure wound therapy or specialized dressings.
- Regularly document wound assessments, treatments, and progress in patient records.
