Duration: 3 Month Assignment

Salary: $18.50 per hour

Position Summary:

The Community Health Worker (CHW) at SEMHA will play a key role at the Kids’ Health Connection program, serving as a front-line resource for families with children ages 0–4, that focuses on addressing early childhood health needs and reducing healthcare disparities across the metro-Detroit area. The CHW serves as a trusted community liaison, working closely with families, healthcare providers, insurance payors, and community organizations to improve access to care, promote healthy development, and support overall family well-being. This is a 3-month grant funded part-time assignment with benefits, that may be extended.

Key Responsibilities:

  • Partner with families, healthcare providers, schools, and community agencies to ensure coordinated, high-quality care and avoid duplication of services
  • Educate families on appropriate emergency room usage, well-child visits, immunizations, and preventive healthcare practices
  • Assist families in accessing essential resources, including transportation, housing, food assistance, and behavioral health services
  • Support families in understanding care plans and medical instructions
  • Encourage and empower families to actively participate in their health and well-being
  • Coordinate appointments and facilitate access to healthcare services
  • Engage community members to gather feedback and improve program effectiveness
  • Maintain accurate, confidential participant records
  • Attend required training and maintain program compliance
  • Demonstrate professionalism in all interactions

Qualifications:

  • Community Health Worker Certification issued by the State of Michigan (required)
  • Valid driver’s license, current auto insurance, and reliable transportation (required)
  • Ability to attend all required program trainings

Education & Experience

  • High school diploma or equivalent required; Associate or higher preferred
  • Minimum of two (2) years of relevant work experience
  • Experience in a healthcare or community-based setting strongly preferred
  • Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) and general computer skills
  • Experience with digital engagement tools (e.g., Canva, Instagram, Facebook) preferred
  • Strong organizational skills with attention to detail and documentation accuracy
  • Self-motivated with the ability to work independently and manage multiple priorities
