Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Macomb, MI providing home health services. You will help patients recover from injuries and manage chronic conditions while working with a caseload that supports functional independence and safe discharge to community settings.

Minimum Requirements

Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Michigan Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience with Home Health preferred; candidates new to home health will be considered.
Familiarity with Home Care Home Base EMR preferred.
Ability to meet productivity expectation of approximately 30 points per week.
Valid driver’s license and reliable transportation. Mileage reimbursed at IRS standard mileage rate.

Preferred Qualifications

Prior home health caseload management experience.
Comfort performing home safety assessments and working independently in patient homes.
Experience with telehealth or remote documentation workflows.

Assignment Details

Shift days: M-F.
EMR: Home Care Home Base.
Interview format: Teams/Video Call.
Caseload geography: primary hub in Macomb with travel expected within roughly a one-hour radius depending on referral patterns.
On call: None.
