- Travel assignment providing direct, site-based nursing care for participants in a home health setting

Responsibilities
- Provide direct nursing care to participants during home visits, applying advanced nursing knowledge independently
- Triage emergent situations and coordinate appropriately with PCPs
- Monitor acute and chronic medical conditions in collaboration with Primary Care Providers
- Implement and adjust care plans, administer treatments, procedures, and medications as prescribed
- Conduct wound care, clinical assessments, lab draws, and other nursing tasks as ordered
- Communicate effectively with participants, caregivers, and care teams to ensure safe, quality care

Requirements
- Active Registered Nurse license in Michigan
- Education: Associate degree in Nursing required; Bachelor’s degree preferred
- CPR certification by American Heart Association (or willingness to obtain)
- Minimum of one year of experience with frail, elderly, or long-term care population
- At least one year of home care experience preferred; wound care experience preferred
- Reliable transportation with a valid Michigan driver’s license and car insurance
- Must be 18 years of age or older
- Vaccination requirements or willingness to be fully vaccinated against influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B
- Ability to work in community and home settings with potential interruptions and need for flexible scheduling
