Position: Diabetes Educator/Foot Care RN - Health Services

Dual Site

Pay Grade: NE13 Status: Non-Exempt Background Check: Extensive

Hiring Range: $25.87 - $31.84 - $37.79 Commensurate with Verified Qualifications.

Summary: This position is responsible for providing professional compassionate diabetes and foot care services provided by the Little River Band of Ottawa Indians Tribal Health Services from their two clinics. Under the guidance of the Clinic Supervisor, the Diabetes Educator/Foot Care RN provides education on diabetes management, assists patients with their care plans, provides comprehensive foot assessments, fosters a supportive environment and collaborates with the healthcare team to ensure optimal comprehensive care for patients with diabetes.

Employment Qualifications:

  • Associate's degree or higher in Nursing; and
  • A current registered nurse licensure in the State of Michigan; and
  • Work experience of two (2) or more years as a diabetic care, wound management, or foot care nursing; and
  • Certified Diabetic Educator (CDE); and
  • Diabetic Foot Care certificate; and
  • A valid Drivers License with the ability to be insured under the companys insurance policy.

Duties and Responsibilities

  1. Provides exceptional customer service while receiving and assisting patients. Assists providers in developing and administering patient care plans.
  2. Provides evidence-based education related to diabetes and the disease process; medication, diet, physical activity and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection.
  3. Develops and implement educational programs for diabetic patients and families.
  4. Advises patients on long term complications.
  5. Provides diabetic resources and consultation to the clinical team.
  6. Assesses, monitors and implements treatment plans for patients at various stages of the disease; conducts regular follow-ups with patients.
  7. Conduct comprehensive foot assessments for patients with diabetes, identifying potential risks such as neuropathy, ulcers, or circulation issues.
  8. Provide safe and compassionate foot care, including nail trimming, skin care and management of corns, calluses and ingrown nails.
  9. Document assessment, interventions and plans including recommendations and goals, progress notes and discharge summaries.
  10. Maintains strict adherence to infection prevention and control practices, including proper instrument cleaning and sterilization.
  11. Develops program goals and objectives, reports and manages Tribal specific data related to the diabetic registration for internal planning
  12. Coordinates with other tribal and community services to enhance access of services and resources.
  13. Maintains professionalism and confidentiality in all aspects of work and complies with all applicable tribal, federal, or state laws and procedures.
  14. All other position/department related duties as assigned by supervisor.

Physical Demands

Sitting or standing for long periods of time, frequent walking, and occasional lifting, reaching, bending, kneeling, or stretching is to be expected. The nature of the work also involves manual dexterity to operate office equipment and while keyboarding. An employee will need normal, or corrected to normal range of sight and hearing and will routinely carry work related materials up to twenty-five (25) pounds. Work may involve exposure to infections, communicable diseases, contaminated materials, bodily fluids, and radiation resulting in the need to wear protective equipment such as masks, eye protection, rubber gloves, and gowns.

Working Environment

Work is performed in a climate controlled medical office environment. The employees work schedule may frequently need to be flexible and adjusted according to business need. The employee must be able to effectively handle stressful and compromising situations while remaining focused and professional, as well as be able to deal with difficult people without losing perspective. Occasional overnight travel may be necessary to attend meetings or trainings.
