VNA NP PALLIATIVE CARE
The Nurse Practitioner for Palliative Care Services is responsible for collaborating in the development of Covenant HealthCare’s Palliative Care Program and the provision of Palliative Care services within the community and organization. This person demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare.
Responsibilities:Provides inpatient and outpatient palliative care services as part of an interdisciplinary team. Facilitate clarification of patient and family goals of care. Consults with the attending physician and the interdisciplinary team to establish a written plan of care at intervals specified in the plan. Facilitates access to appropriate supportive care services. Provides education to staff and coworkers. Provides appropriate documentation of patient care services to support professional billing for these services. Participation in on-call coverage. Participation in interdisciplinary team meeting to review active cases as necessary. Assists in the development of standards of care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Other duties as requested/assigned.
Qualifications:EDUCATION/EXPERIENCE REQUIREMENTS Current State of Michigan licensure as a Nurse Practitioner required. One (1) year experience working as part of an interdisciplinary team required. Demonstration of education/CME specific to Palliative Care required. Board certified or eligible in Hospice/Palliative Medicine preferred. Two (2) to three (3) years’ experience practicing in Palliative Care preferred. Palliative Care of Advance Illness Management experience preferred. KNOWLEDGE/SILLS/ABILITIES Knowledge of healthcare systems, policies and procedures. Demonstrates Leadership skills. Ability to communicate effectively both verbally and in writing. Ability to understand regulations concerning ethical health care practices. Analytical skills in order to gather information, reach appropriate conclusions and make recommendations. Experience in Program Development (preferred). Experience in Quality Improvement. Demonstrates ability to establish and maintain effective interpersonal relationships and communication. Demonstrates ability to accept and implement change. Demonstrates ability for problem solving. Strong organization skills with the ability to work independently and take initiative. WORKING CONDITIONS/PHYSICAL REQUIREMENTS Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards. Constant sitting, talking, or hearing. Frequent standing, walking, lifting, carrying, and use of hands to finger, handle and feel. Frequent lift 0-25 lbs. Frequent near, far, depth perception, color vision, and field of vision. Occasional push, pull, balance, climbing, stooping, crawling, kneeling, crouching, taste or smell. Occasional lift 26-50 lbs.
