Job Details
- Contract Length: 13 weeks
- Start Date: July 14, 2025
- Shift: Days
- Shift Duration: 8-hour days
- Shifts per Week: 5
- Weekly Hours: Typically 40 hours/week (36 hours guaranteed)
- Estimated Weekly Pay: $1500 - $3540
- Minimum 1+ year experience as a Physical Therapist
- Minimum 6+ months experience in a Home Health setting
- OASIS and HCHB experience required
- Current licensure in the state of practice
- Current CPR certification
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- Provide high-quality physical therapy services to patients in their homes
- Develop individualized care plans based on patient needs
- Collaborate with healthcare professionals to ensure comprehensive patient care
- Document patient progress and adjust treatment plans as necessary
