Experience

• 12 months of role experience is required with some in the last 12 months.
• Must have at least 12 months of Home Health experience with 12 months within the past 3 years.
• Experience with Oasis is required from any number of months.
• Experience with Homecare Homebase (HCHB) is required from any number of months.

Requirements

• Candidates must have a Michigan license (required for submission).
• Local & travel allowed.

Additional Details

Shift & Scheduling: Monday - Friday. Must meet minimum productivity expectation of 30 points per week. 1 weekend every 4 weeks and 2 holidays per year - one during summer and one during winter.

Required Skills/Experience: Must have at least 12 months of home health experience within the past 3 years. OASIS E and hands-on HCHB experience required

Floating Requirements: Area of coverage primary includes Clawson, Franklin, Hazel Park, Southfield, Pleasant Ridge, Huntington Wood, Southfield, Fraser, Ferndale, Oak Park, Centerline, and Warren. Secondary areas include Royal Oak, Madison Heights/Clawson, Berkley, Royal Oak, Troy, Sterling Heights, and Detroit (only in zipcodes 48203 and 48234)
