Home Health Speech-Language Pathologist (SLP)

Part-Time | 2 Days per Week

Service Area: Bay City • Midland • Saginaw

What You’ll Do

• Complete in-home evaluations for speech, language, cognition, voice, and swallowing

• Develop and implement individualized treatment plans aligned with physician orders

• Educate patients and caregivers on communication strategies and home exercise programs

• Collaborate with nurses, therapists, and physicians to ensure coordinated care

• Document services accurately and in a timely manner

• Support patient safety and help reduce unnecessary hospitalizations

What We’re Looking For

• Master’s degree in Speech-Language Pathology from an accredited program

• Active state SLP license (or Educational Limited License with supervision)

• Prior SLP experience preferred; home health experience a plus

• Strong organizational skills and ability to work independently

• Compassionate, patient-centered approach to care

• Reliable transportation and valid driver’s license
