As a Case Manager for Population Health, you will be an integral member of a patient-centered care team. You will engage in collaborative practices, including assessment, planning, facilitation, care coordination, evaluation, and advocacy. Your role is vital in meeting the comprehensive health care needs of individuals and families, ensuring patient safety, quality of care, and cost-effective outcomes. You will address the needs of patients experiencing critical events or diagnoses requiring complex management strategies across ambulatory, inpatient, or health plan settings.

Responsibilities

  • Conduct comprehensive assessments of patient and family/caregiver's biomedical, psychological, social, and functional needs.

  • Develop personalized patient-centered care plans to optimize the patient's care experience.

  • Engage patients and their families as part of the care team through advocacy, ongoing communication, health education, and resource facilitation.

  • Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to assist patients in overcoming barriers.

  • Coordinate seamless and safe transitions between care settings for patients.

  • Advocate for appropriate delivery of services within the patient's health plan benefit structure.

  • Collaborate with appropriate members of the patient's treatment/care team to co-manage patients with complex medical and social needs.

  • Continually evaluate the patient's response to the care/treatment plan, making necessary modifications.

  • Plan and participate in process improvement activities designed to reduce risk.

  • Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual assault.

  • Support department-based goals contributing to the success of the organization.

  • Perform other duties as assigned.

Essential Skills

  • Strong computer skills and knowledge.

  • Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care.

  • Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare and Medicaid regulations, and case management principles.

  • Understanding of social determinants of health and their impact on a patient's wellbeing.

  • Strong organizational, planning, and implementation skills.

  • Strong sense of compassion with the ability to successfully advocate for patients and their families.

Additional Skills & Qualifications

  • Minimum 5 years acute inpatient case management experience with acute discharge planning.

  • MLSW certification.

  • BLS certification through AHA.

  • Experience using CarePort system.

  • Preferred experience charting in AllScripts, GEMS/Sunrise.

  • Willingness and ability to be on the floor with patients and acclimate quickly.

Work Environment

The position operates within a Monday to Friday schedule, from 7:30 am to 4:00 pm, with a requirement for weekend rotation. The role involves direct interaction with patients across various care settings, requiring the ability to quickly adapt and respond to patient needs.

Job Type & Location

This is a Contract position based out of Grand Blanc, MI.

Pay and Benefits

The pay range for this position is $35.00 - $55.00/hr.
