University Home Care Inc is seeking a compassionate and dedicated Llmsw’s hospice professional to join our team in Livonia, MI. This role involves providing exceptional hospice care and support to patients and their families, ensuring comfort and dignity during end-of-life care.

• Provide clinical social work services to hospice patients and their families in accordance with regulatory and agency standards.
• Conduct psychosocial assessments and develop individualized care plans.
• Collaborate with an interdisciplinary team to coordinate patient care and support.
• Offer counseling and emotional support to patients and families coping with terminal illness.
• Document patient interactions and care plans accurately and timely.
• Ensure compliance with all relevant laws, regulations, and University Home Care Inc policies.

• Licensed Master Social Worker (LMSW) with hospice experience preferred.
• Strong knowledge of hospice regulations, psychosocial assessment, and counseling techniques.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver’s license and reliable transportation for patient home visits.

• Competitive hourly compensation: $24.00 - $65.00, paid semi-monthly.
