University Home Care Inc is seeking a compassionate and experienced Registered Home Care Nurse with Kinnser experience to provide high-quality care to patients in Southgate, MI and surrounding areas. This role involves delivering personalized nursing care in the comfort of patients' homes, ensuring their health and well-being while promoting independence.
Responsibilities- Conduct comprehensive assessments of patient health status in home settings.
- Develop and implement individualized nursing care plans based on patient needs.
- Provide skilled nursing services such as wound care, medication management, and chronic disease management.
- Document patient care accurately and timely using Kinnser software.
- Communicate effectively with patients, families, and interdisciplinary healthcare teams.
- Maintain compliance with all regulatory and company standards.
- Current Registered Nurse (RN) license in the state of Michigan.
- Proven experience with Kinnser software or similar home health documentation systems.
- Strong clinical skills in home health nursing.
- Excellent communication and interpersonal skills.
- Reliable transportation and ability to travel to patient homes in Southgate and surrounding areas.
- Ability to work independently and manage time efficiently.
- Competitive pay rate of $65.00 to $100.00 per visit.
