$45-$50 per hour plus mileage reimbursement | Paid Weekly

Position Overview

Under general supervision of the nursing supervisor, utilizes the knowledge and application of nursing principles to provide direct patient care in the home. Assesses need for other allied health care services such as physical therapy, occupational therapy, nutrition, speech pathology and social work, to ensure an integrated home health care delivery system. Performs other related duties as required.

Starts off as a 17-week contract with strong potential to convert based on performance

Schedule

• Monday–Friday | 8:00 AM – 4:30 PM
• One Saturday required each month (rotation)

Essential Responsibilities

• Candidates will be supporting the Home Health department with the patient load in Wayne, Oakland, and Macomb counties. They should anticipate a case load of approximately 6 patient visits per day with an average of 40-60 miles driven per day .
• Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate individualized Plans of Care (POC).
• Incorporate physical, psychological, social, spiritual, and cultural factors into patient care planning.
• Maintain accurate, up-to-date clinical documentation for all visits, contacts, and care updates.

Required Skills & Competencies

• 1+ year of previous Home Health Nursing experience
• Active Michigan RN Licensure
• Active BLS Certification from the AHA
• Ability to effectively communicate with patients, families and hospital personnel.
• Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.
• Must be able to lift, carry, transport and move clients, if needed.
• Valid driver’s license and clean driving record
• Reliable transportation with insurance

Preferred

• 2+ years of acute care experience preferred
• Bachelor of Science in Nursing (BSN)
• Oasis and Homecare Homebase experience preferred

Education

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (Required)
• Bachelor of Science in Nursing (Preferred)
