Responsibilities
- Conduct thorough patient evaluations and develop individualized plans of care following physician prescriptions
- Implement physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
- Monitor progress, adjust treatment plans, and document outcomes in patient records
- Educate patients and families on exercises, safety, and discharge goals; coordinate discharge planning and return-to-work activities
- Collaborate with the rehabilitation team to ensure coordinated, patient-centered care
- Provide one-on-one therapy sessions and manage on-call coverage as required

Requirements
- Active Michigan physical therapy license
- Graduate of an accredited PTA program (required)
- BLS (AHA) certification
- Experience: 1 year of swing bed, acute and outpatient PT experience required; orthopedic experience preferred; travel experience preferred
- Ability to work in inpatient/outpatient rehabilitation environments
- Documented ability to collaborate effectively with a multidisciplinary team
