NOW HIRING: RN Case Manager – Field & Telephonic Based

Location Requirement:

• Michigan residents only
• Primary service area: Wayne County & Macomb County, MI
• Candidates must live in or very near these counties and be willing to travel within the region

Schedule

• Monday–Friday | 8:00 AM – 5:00 PM EST
• Training follows the same schedule

Position Summary

The RN Case Manager utilizes a collaborative, member-centered approach to assess, plan, facilitate, and advocate for care options and services. This role supports members by leveraging clinical expertise, benefit plan knowledge, and internal/external resources to promote optimal, cost-effective health outcomes.

This position is a hybrid of field-based and telephonic work , requiring strong clinical judgment, organization, and communication skills.

Key Responsibilities

• Conduct comprehensive clinical assessments using data review and clinical tools
• Evaluate member eligibility, benefits, and available programs/services
• Develop and implement standardized case management plans
• Apply and interpret clinical criteria, guidelines, policies, and regulatory standards
• Coordinate care through collaboration with members, providers, and care teams
• Advocate for appropriate services and effective use of benefits
• Ensure compliance with regulatory, accreditation, and quality management requirements
• Maintain accurate documentation across multiple systems

Required Qualifications

• Active, unrestricted RN license in the state of Michigan
• 3+ years of clinical practice experience , such as: hospital setting, home health, Ambulatory or alternative care settings; Healthcare and/or managed care industry experience
• Strong computer proficiency, including navigating multiple systems and keyboarding
• Case Management experience required
• CCM (Certified Case Manager) certification
