Job responsibilities:
- Work 36 hours per week with 12-hour shifts.
- Coordinate care activities with the interdisciplinary behavioral health team.
- Provide patient education related to the treatment plan and follow-up resources as directed.
- Document observations, interventions, and patient response in the medical record.
- Perform mental status assessments and monitor behavioral health symptoms over time.
