Physical Therapist - Travel Contract
Looking for a change of scenery and a chance to help others. Join a home health Physical Therapist assignment in Flint, MI where you will provide skilled physical therapy services to patients in their homes. You will plan, implement, and evaluate individualized plans of care for patients across the lifespan. Collaborate with an interdisciplinary team, document clinical care, instruct patients and families in transfer techniques and equipment use, and apply infection control and safety practices.
Minimum Requirements
- Current state license as a Physical Therapist required to start the assignment.
- Bachelor of Science in Physical Therapy (or Master’s/Doctoral degree) from an APTA‑approved program required.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and active auto insurance required.
- 1–2 years experience as a Physical Therapist in acute care or rehabilitation setting required.
- Ability to perform physical demands of home health care including lifting, bending, prolonged standing and walking.
- Ability to work a flexible schedule and travel locally as needed.
- Must read, write, and speak fluent English.
- Client requirement: Documentation and completion of Plan of Care (POC), progress notes, phone orders, and other clinical records on an ongoing basis.
- Client requirement: Ability to assess home environment, order and adapt durable medical equipment to meet patient needs.
- Client requirement: Demonstrated knowledge of durable medical equipment.
Preferred Qualifications
- Previous home care experience preferred.

Impacting Patient Care
Provide direct care that restores or maintains patient well‑being in the home setting. Perform clinical assessments, develop and revise Plans of Care, and coordinate with physicians and interdisciplinary teams. Educate patients and families on therapeutic procedures, safe transfers, and equipment use. Supervise other team members when applicable and apply infection control and universal precautions.
