Our Intensive Care Coordination with Wraparound (ICCW) program supports children, youth, young adults, and families with complex needs through a collaborative, family-driven approach. This community-based role blends Wraparound planning with comprehensive care coordination, allowing Care Coordinators to work directly with families and community partners to create meaningful, individualized plans. This position requires a bachelor's degree in a Human Service field.
