University Home Care is seeking a skilled and compassionate Physical Therapist to join our team. In this role, you will be responsible for providing high-quality physical therapy services to patients in their own homes. As a Physical Therapist with Home Care, you will play a vital role in improving patients' mobility, relieving pain, and enhancing their overall quality of life.

• Evaluate patients' medical history, assess their physical condition, and develop appropriate treatment plans.
• Provide hands-on therapy, exercises, and interventions to improve patients' strength, flexibility, and mobility.
• Instruct patients and their families on proper techniques for exercises and activities to be performed at home.
• Document patients' progress, maintain accurate medical records, and communicate with other healthcare professionals as necessary.
• Collaborate with other members of the healthcare team to ensure comprehensive and coordinated care.
• Stay up-to-date on the latest advancements in physical therapy techniques and modalities.

• Valid and current license to practice as a Physical Therapist in the state of Michigan.
• Minimum of 2 years of experience as a Physical Therapist, preferably in a home care setting.
• Strong knowledge and understanding of physical therapy principles, techniques, and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and prioritize tasks effectively.
• Valid driver's license and reliable transportation.
