Our Home Home Care is a growing home care agency dedicated to providing compassionate, high-quality in-home support to seniors and individuals with disabilities. We believe in building strong relationships — not only with our clients and caregivers but also with our partners in the community.

We are seeking a proactive and personable Community Liaison to help expand our presence and visibility within the local healthcare and senior services community. This role is perfect for someone who is passionate about helping others, understands the importance of community partnerships, and enjoys networking and relationship-building.

• Represent Our Home Home Care at local hospitals, clinics, senior centers, rehabilitation facilities, discharge planning teams, and community events

• Build and maintain strong relationships with social workers, discharge planners, case managers, physicians, and other referral sources

• Educate community partners and potential clients about the services our agency offers

• Identify new referral sources and maintain a contact database

• Support the onboarding of new clients by coordinating with agency staff to ensure smooth transitions

• Help develop outreach strategies, brochures, and marketing materials

• Monitor client satisfaction and serve as a communication bridge when necessary

• Track referrals, outcomes, and maintain accurate records of outreach efforts
