Key Responsibilities:

  • Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans.
  • Provide emotional support and education to patients and families about end-of-life care and self-care techniques.
  • Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress.
  • Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care.
  • Maintain accurate clinical documentation and participate in performance improvement initiatives.

Requirements

  • Current Michigan Registered Nurse (RN) License in good standing.
  • Minimum of 1 year experience in hospice or palliative care.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team environment.
  • Empathetic and compassionate approach with patients and families.
  • Willingness to participate in on-call duties as needed.
