Position Summary:

The liaison, reporting to the Business Development Manager, is responsible for establishing and sustaining referral relationships with healthcare providers, facilities, and community partners to promote Axiom Cares Home Health services. This role serves as the primary point of contact between the agency and referral sources, ensuring growth while adhering to ethical marketing standards and maintaining regulatory compliance.

Key Responsibilities:

• Build and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living facilities, group homes, case managers, social workers, discharge planners and community organizations.
• Serve as the primary liaison between referral sources and Axiom's administrative team.
• Educate referral partners on agency services, admission criteria and coverage areas.
• Meet regularly with the Business Development Manager for assistance with the position as well as to track progress toward company goals via metrics.

• Strong interpersonal, communication and presentation skills.
• Ability to build long-term relationships.
• Self-motivated with excellent time-management and organizational skills.
• Ability to work independently in the field.
• Proficient in Microsoft Office & Google Sheets.

Work Requirements:

• Frequent travel within assigned territory (currently predominantly Wayne County).
• Ability to attend meetings in person, virtually and via phone, as well as attend community events and presentations.
• Valid State of Michigan driver license and reliable transportation.

Performance Metrics:

• Referral volume and growth
• Admission conversion rates
• Referral source retention
• New referral source development

Qualifications:

1. At least one year experience in sales preferred.

2. At least two (2) years of experience in health care marketing preferred.

3. Ability to market aggressively and deal tactfully with customers and the community.

4. Demonstrates excellent communication, negotiation and public relations skills.

5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in

performing job responsibilities.
