RN Case Manager Job Details

• Assignment Length: 13 weeks (Travel)
• Location: On-site in Jackson, MI (Zip: 49201) – Michigan healthcare jobs
• Shift: 8-hour day shifts (8:00am – 4:30pm), Monday through Friday
• Hours per Week: 40 guaranteed hours
• Employment Type: Travel RN
• Hourly Pay Rate: $46–$59/hr
• Estimated Weekly Salary: $2000–$2050

RN Case Manager Qualifications

• Active and unrestricted Registered Nurse (RN) license in Michigan
• Bachelor of Science in Nursing (BSN) required
• Minimum 2 years of RN case management experience (utilization case management not applicable)
• Strong interpersonal, communication, and negotiation skills
• Proficiency in data management and PC skills
• Excellent organizational and time management abilities; able to prioritize multiple tasks independently
• Preferred: Experience with Care Manager and Epic systems, discharge planning, and at least 3 years of RN clinical experience

RN Case Manager Responsibilities

• Collaborate with physicians and the interdisciplinary healthcare team to facilitate patient care
• Enhance quality of clinical outcomes and patient satisfaction while managing cost of care
• Integrate and coordinate utilization management, patient education, care facilitation, and discharge planning
• Manage a designated patient caseload, planning effectively to meet patient needs and promote efficient resource utilization
• Facilitate precertification and payor authorization processes
• Manage patient care across the continuum, intervening to remove barriers to timely and efficient care delivery
• Apply process improvement methodologies to evaluate outcomes of care
• Coach and support clinical documentation efforts, ensuring accuracy and completeness
• Coordinate communication with physicians and serve as a clinical resource for coders
• Demonstrate knowledge and skills appropriate to the patient population served, including principles of growth and development across life cycles
