Position Purpose: Serves as both a peer mentor and a connector. Drawing from lived experience and a working knowledge of community systems, ensuring people have the relationships, tools, and care they need to build a life in recovery.
Principal Responsibilities:
- Act as a role model to individuals seeking or maintaining recovery, offering hope and demonstrating that long-term recovery is possible.
- Provide one-on-one coaching support; help individuals identify goals and strengths through the development of a personalized Recovery Plan.
- Facilitate group sessions that foster peer connection, skill-building, and shared accountability in recovery.
- Create a welcoming, inclusive atmosphere at the Recovery Center, engaging all who walk through the door, whether scheduled or dropping in.
- Support new participants through "Welcoming Sessions," orienting them to Ten16’s services and values.
- Offer ongoing encouragement and re-engagement support when individuals face setbacks or disengagement.
- Collaboratively develop strength-based recovery service plans that reflect the individual’s goals, needs, and priorities.
- Help participants navigate and access community services such as housing, food, healthcare, employment, education, and peer supports.
- Maintain regular contact (in-person or by phone) to monitor progress, celebrate milestones, and support continued movement toward goals.
- Coordinate with external providers (e.g., treatment agencies, hospitals, primary care, and recovery support services) to ensure consistent care and communication.
- Assist participants in building up natural supports and strengthening their recovery capital.
- Maintain professional relationships with local resource providers and social service agencies to facilitate effective referrals.
- Accurately document all services and encounters in a timely and professional manner.
- Ensure that all recovery coaching and case management services are clearly reflected in documentation, aligned with program standards, and entered into the EMR and ready for billing.
- Participate in team meetings, case consultations, and staff development activities to ensure a collaborative and informed approach to care.
- Contribute to a Recovery Center culture that values connection, dignity, and empowerment.
Principal Organizational Relationship:
- Reports to the Recovery and Wellness Manager
Preparation, Experience and Skills:
- Three to five consecutive years of quality, active recovery from an addiction. If a former client of Ten16, must have 2 years from the last date of service to be considered for a position.
- Education is a secondary consideration. Preference would be associate or bachelor’s degree in human services. Life/work experience given strong consideration
- CCAR certification is required no later than one year from the date of employment, but preferably within 6 months, if not already secured.
- Community Health Worker (CHW) credential earned within the first year of employment, if not already secured.
- Excellent people skills; experience in outreach, networking, and helping others.
- Strong professional boundaries; willing to make changes in community interactions recognizing dual relationships that will be created working as a recovery coach.
- Demonstrated understanding of confidentiality.
- Good written communication skills
- Solid computer and time management skills; must be sufficient to meet the documentation requirements and productivity goals of the position.
- Strong orientation toward a team approach to service delivery.
