General Statement:
Under the direction of the Clinical Operations Manager and within established policies and procedures, the RN Care Coordinator leads GCHC’s disease management and disease prevention programs. Provides assistances to providers and other care team members and works directly with patients. This position assists with mobile services and rotates at all GCHC locations.. Disease management and prevention activities include the development of individualized care plans that are patient-centric, ensure quality care using evidence-based clinical and prevention guidelines and promote efficiency in the delivery of health care.
Minimum Requirements:
• Graduation from an accredited school of Nursing;
• Current license as a registered nurse (RN) in the State of Michigan;
• Ability to move up to 30 lbs. in order to transport supplies/equipment and to move or position patients;
• Two years working for a physician or other outpatient health center/clinic setting;
• Proficiency with Electronic Health Records.
Preferences:
• Experience working with vulnerable populations such as individuals who are homeless, have a mental illness, those with addiction disorders, and chronic conditions;
• Working for Community Mental Health and/or Federally Qualified Health Center;
• Care management experience;
• Experience in continuous quality improvement;
• Proficiency with Microsoft Suite (Word, Excel, Outlook).

- Lifting, Standing, and Walking With or Without Reasonable Accommodations:
- Position requires the ability to lift up to 35lbs on an occasional basis
- Position requires the ability to regularly stand for extended periods of time
- Position requires the ability to walk at a minimum of 35% up to a maximum of 65%
- Position will require the ability to occasionally stoop, bend, or reach to perform work tasks with or without reasonable accommodation
- Position must be able to perform tasks requiring manual dexterity, including the use of hands and fingers for handling, grasping, typing, writing, or operating equipment with or without reasonable accommodation.
